Home Grown Connections

NDIS Self-Managed
If you self-manage your child’s NDIS plan, Home Grown Connections will send you a service invoice through SquareUp. Once you’ve paid it, we’ll forward the paid invoice back to you so you can submit it directly to NDIS for reimbursement.

Plan-Managed Clients
If your child’s plan is managed by a Plan Manager, Home Grown Connections will send the invoice straight to them for payment.

Service Agreement
Families using NDIS are encouraged to set up a Service Agreement with us. This simply outlines how much funding you’d like to use with Home Grown Connections and the services you’d like to access. We’ll go through this together during your assessment or your first session.

Ways you can use your NDIS funding with Home Grown Connections:

• Increased Social & Community Participation: Can be used for group programs that help children join in social and adventure activities.
• Capacity Building – Daily Activities: Can be used for assessments, reports, one-on-one therapy sessions, or group programs.
• Capacity Building – Improved Relationships: Can be used for parent or caregiver training, behaviour therapy, and behaviour support plans.

The current NDIS session fee is $210.16, which includes both direct therapy and non-direct support time. The fee and travel rate for NDIS participants are as per current pricing guidelines.

Enhanced Primary Care Plan or Chronic Disease Management Plan

Enhanced Primary Care Plan (EPC) / Chronic Disease Management Plan (CDMP)
Your child may be able to access this plan without needing a formal diagnosis. It can be set up by your GP.

If your doctor decides your child is eligible, they can provide up to 5 subsidised Occupational Therapy sessions each calendar year.

Eligibility is usually based on whether your child has a complex or ongoing condition that requires support from a team of professionals. This may include specialists such as a Speech Pathologist, Psychologist, Paediatrician, Dietitian, or Physiotherapist, alongside Occupational Therapy.
